Ready to book?
You may be ready to schedule your initial interview. If so, let Dr. Cook know you are interested in becoming a client, and provide him with some information about yourself. You can do this in a variety of ways. One is to use the Aegis contact form. Another is to call Dr. Cook at 250-881-1206. Alternatively, you may wish to sign up on the booking service at GoSeeJohn.com so that Dr. Cook can send you a link for completing the application intake form. Once you and Dr. Cook are satisfied that there is a good match between what you seek and what he can provide, he will send you a link to the registration intake form, and enable the self booking feature. This will give you full online access to appointment times: Monday through Friday, from 2:00 until 5:30 PM.
How to register
Once you have been accepted as a client at Aegis, the next step is to register. Registration is our way of learning a little about you, and giving you a bit of information about what to expect, before we see you. There are three alternatives for registering.
- Print out the forms below and bring them completed to your first session.
or - Arrive 15 minutes before your first session and fill them out from the clipboard on the reception counter.
or - Complete the online intake form available on the booking service at GoSeeJohn.com.
